Our physicians accept the majority of healthcare plans. 

Before your arrival, please review your insurance benefits.  Some insurance plans require a referral from your primary care provider before they will cover your visit with our physicians.  To determine your insurance benefits and eligibility, please contact your insurance carrier directly before you arrive for your appointment.  Our office needs to know all pertinent insurance information including the name and address of your insurance carrier, your insurance identification number and the insurance subscriber's name and date of birth if not the patient.  It is important you bring your current insurance card(s) to each visit and inform us of any changes in your coverage. 

 

Billing & Payments


We will make every effort to verify your insurance coverage prior to your appointment, but we cannot guarantee coverage of services by your insurance company.  We will bill your insurance on your behalf, but full payment is ultimately your responsibility.  Please note that you are responsible for payment of all services that are not covered by your insurance plan.  Any patient balance after insurance is normally due within 30 days, but we can set up a payment contract for outstanding accounts.  We also request co-payment for office services at the time of your visit unless other arrangements have been made.
